He probably didn't practice proper safety procedures like wearing a bell around ones neck when going down tunnels to work on pipes.
You aren't doing it wrong if no one knows what you are doing.

This is a good example of where all those onerous job-killing regulations come from. Some huge company or companies do something that is, by all reasonable evaluation, awful and something that could be fixed/avoided very easily, but there isn't any way to make them see the light or the evil of their ways. So some Congressman who knows somebody personally who got hosed introduces a bill requiring lenders to offer a "grace period" and facilitate changing of ownership of the mortgage without penalty. Then the banks and people scream about fedral innerference with free ennerprise or some such drivel.
