The horror stories we are hearing (that small percentage of TRUE ones, that is) cannot be shown to be more than insurance companies taking advantage of yet another opportunity to gouge the fuck out of their captive audience. The "business decision" was to comply with government regulations, which outlawed previous policies that did not have the "essential benefits" of ACA. Many policies lost their "grandfather" status which would have exempted them, by making changes since 2010 in copays, benefit coverage, etc. Bottom line is that the ACA invalidated most private non employer-provided policies forcing new policies to be written that included the "essential benefits", some of which many people didn't want such as maternity benefits for men. In addition to very low reimbursement, providers are put at risk for getting paid at all. Under a provision of ACA, insurers are required to allow a three-month grace period of nonpayment of premiums before discontinuing coverage for enrollees in exchange plans who qualify for the federal premium tax credits. But under final regulations issued in March 2012 by CMS to implement the provision, insurers in health exchanges are required to pay claims arising in the first month only after an enrollee fails to make a required premium payment. The insurers are allowed to “pend” claims during the second and third month, and ultimately deny them, thus giving insurers the power to retroactively terminate coverage for the final two months of the grace period, putting the providers at risk for non-payment for services rendered.
Imagine that.... insurance companies using any excuse to raise rates. Ultimately what matters is that it helps more people that it hurts, that remains to be seen. I've always said universal care, but remember republicans wouldn't let anything but this pass.... wonder why?But the Department of Health and Human Services then wrote regulations that narrowed that provision, by saying that if any part of a policy was significantly changed since that date -- the deductible, co-pay, or benefits, for example -- the policy would not be grandfathered.

Insurers may be happy that forced enrollment in the exchanges is being pushed back for a bit. But over the long term, insurers actually want to strengthen the individual mandate, and have been lobbying the White House to that end. They’re concerned that the individual mandate is too weak.
